Position: Health Services Coordinator

Description: Coordinate, supervise, and administer the health services provided to consumers and employees.

Requirements: Bachelors of Science in Nursing degree, current Wisconsin licensure as an RN, at least two years experience in psychiatric, home health care, rehabilitative, or community health nursing. Must pass care giver background check as mandated by Wisconsin Chapter HFS 12.

Skill Requirements: Have the ability to perform “major life activities??? as described in the EEOC laws, Chapter 29 CFR Section 1630. Effective oral and written communication necessary in English. Knowledge of psychiatric nursing and treatment modalities including psychotropic medications preferred. Prior experience working with persons with a developmental disability, mental health diagnosis, and traumatic brain injury preferred. Prior experience working with individuals in their own home and/or community health helpful. Proficiency in Microsoft Office products preferred. Ability to lift 25 pounds.

Driving License Requirement: This position requires a valid Wisconsin Motor Vehicles License and the individual must meet the BHCS qualification criteria to operate company vehicles as detailed in the Policy and Procedure Manual.

Principle Duties & Responsibilities:

1. Coordinate and supervise the self administration and administration of the prescription and over the counter medication system as directed by physician order ; (E)

2. Coordinate consumer admissions and health assessments as required by Wisconsin Administrative Code HFS 83; (E)

3. Assist the treatment team in the development and implementation of consumer Individual Service Plans, and attend related planning meetings as necessary; (E)

1. Coordinate all consumer psychiatric, health, and dental appointments; (E)

2. Coordinate the provision of all required and optional vaccinations for consumers and employees; (E)

3. Direct all consumer vital signs checks; (E)

4. Direct the disposal of all discontinued, outdated, or unused medications per Wisconsin Administrative Code HFS 83; (E)

5. Direct individualized and/or group consumer nutrition programs; (M)

6. Coordinate the documentation of all health related information contained in each consumer medical record; (E)

7. Coordinate, supervise, and assist in the provision of employee training as mandated by Wisconsin Administrative Code HFS 83 under the direction of the Regional Director; (E)

8. Serve on Corporate and/or Regional committees as assigned; (M)

9. Serve as rights advocate for clients according to Wisconsin Statute sec.51.61(1), and Wisconsin Administrative Codes HFS 83, 88, and 94; (E)

Other Duties:

1. Keep current in field by reading published information and attending authorized seminars and conferences. Periodic out-of-town travel.

2. Other job-related duties as may be necessary to carry out the responsibilities of this position.
