Position: Qualified Resident Care Staff

Description: Under the direction of the Program Manager provide support and training to clients in the activities of daily living, personal health and hygiene, fiscal management, leisure skill development, psycho-social adjustment and symptom management as outlined in the Individual Service Plan (ISP) and the BSI Policy and Procedure Manual.

Requirements: A minimum of a high school diploma or GED. Has successfully completed all training requirements (including minimum initial and continuing education) of Wisconsin Administrative Code HFS 83 and BSI. Must pass caregiver background check as mandated by Wisconsin Chapter HFS 12.

Skill Requirements: Have the ability to perform "major life activities" as described in the EEOC laws, Chapter 29 CFR Section 1630. Effective oral and written communication necessary in English. Ability to perform basic math calculations. Ability to lift 25 pounds.

Driving License Requirement: This position requires a valid Wisconsin Motor Vehicle License and the individual must meet the BSI qualification criteria to operate company vehicles as detailed in the Policy and Procedure Manual. Driving a company vehicle is an essential function of this position.

Principle Duties & Responsibilities:

1. Provide assistance and/or supervision to clients in activities of daily living, rights advocacy, personal care, personal hygiene, meal planning and preparation, fiscal management, leisure activities, and symptom management according to the clients strengths and needs; (E)

2. Assist in the provision of planned care for the clients through implementation of their Individualized Service Plan (ISP) as required by Wisconsin Administrative Code HFS 83 and 88; (E)

3. Participate in and successfully complete all training as required by Wisconsin Administrative Code HFS 83, 88, and BSI policy; (E)

4. Maintain safety and cleanliness in the home as required by HFS 83, 88 and BSI Standards. Assist in the maintenance of the buildings and grounds associated with the facility; (E)

5. Provide an accurate record of client care and behavior; (E)

6. Act as a representative of BSI in a mature and professional manner; (E)

7. Must be able to remain on duty in an emergency until relieved by Supervisor or other facility staff; (E)

8. Assist in the routine maintenance of the vehicles and equipment utilized at the facility; (E)

9. Assist in the yearly inventory of all tools, equipment, furniture, household utensils and furnishings of the facility; (E)

10. Assist Region Nurse and (supervisor title) in the administration of all prescribed and over the counter medications as required by Wisconsin Administrative Code HFS 83; (E)

11. Assist Supervisor in the coordination of all client community appointments and leisure time activities; (E)

12. Transport clients to scheduled appointments and community outings; (E)

13. Under the direction of Supervisor, communicate with other care providers, client case managers, guardians, and family members within the bounds of confidentiality as detailed in BSI policy and procedure; (E)

Other Duties:

1. Keep current in field by reading published information and attending authorized training.

2. Other job-related duties as may be necessary to carry out the responsibilities of this position.
